STEP 2: Complete Registration Form
The downloaded file has blank fields that you can fill in by opening the file in Microsoft Word and simply typing in the information in the blank spaces.
STEP 3: Email form to MTSA
Forward a copy of the registration form to MTSA. Send the email with attachment to jeffrey.cousineau@comcast.net
NOTE: You will receive an email from MTSA with further instructions when your registration form is received.
